2. Complete and sign all pages .
3.  For Corporation Applicant, the signature must be of an eligible officer. If the Applicant is of a partnership business, the signature of all partners of the organization is required.
4. Fax the all pages of the application to:

5. Include a copy of your Sales Tax License.
